There's a Medical University in Plovdiv which, even if it does not provide haemodialysis facilities itself, should be able to steer you in the right direction - the website is available in English - HERE. It appears to be associated with "Ð'ОЛНИЦА СÐ'ЕТИ ГЕОРГИ" (St. George's Hospital), contact details of which are:

As with most things medical in Bulgaria, expect to be treated like the locals (i.e. you'll almost certainly have to pay not insignificant amounts of cash for treatment).
